Manager, Community Health

What you’ll do:
Summary: The Community Health Manager is responsible for the day-to-day coordination and execution of Second Harvest’s health-focused initiatives, including Food is Medicine interventions, healthcare pantry partnerships, and community nutrition outreach. This role ensures programs are implemented effectively, aligned with public health best practices, and responsive to the needs of both partners and neighbors. The ideal candidate is highly organized, detail-oriented, and skilled in managing logistics, tracking data, and meeting grant deliverables. They work closely with healthcare and community partners to coordinate food distribution, deliver nutrition education, and support client-centered pantry models, while also serving as a preceptor for dietetic interns and supporting implementation of Second Harvest’s Nutrition Policy. This position plays a key role in delivering impactful, equity-driven programs that reflect our commitment to health, dignity, and inclusion. Key Responsibilities:
Food is Medicine (FIM) Program Execution (70%)
- Assist in the development and implementation of Food is Medicine interventions, contributing to program design, workflows, and community integration strategies.
- Manage day-to-day operations of FIM interventions, including patient intake, referral coordination, and connection to food and other supportive services.
- Maintain strong working relationships with healthcare partners to ensure effective communication and timely coordination of services.
- Track and monitor program metrics, neighbor feedback, and partner deliverables to ensure quality and accountability.
- Maintain ethical standards and collaborate with healthcare and research partners to ensure compliance with patient privacy and data integrity.
- Submit timely monthly progress reports to internal teams, funders, and external stakeholders.
Healthcare Pantry Management (15%)
- Manage relationships with existing healthcare pantry partners to ensure smooth, ongoing collaboration.
- Coordinate and place monthly food orders for healthcare pantries and clinic partners in alignment with their needs and preferences.
- Support implementation of neighbor-centered pantry models in healthcare settings.
- Collect and track data for healthcare-focused grants and ensure timely reporting.
Nutrition Outreach & Education (15%)
- Coordinate and support the Nutrition Outreach Program, including classes, food tastings, and educational events at partner sites and mobile distributions.
- Develop or adapt easy-to-read, culturally responsive nutrition materials such as handouts and presentations.
- Provide nutrition-related technical assistance to partners and internal teams, including support on menu planning and food box standards.
- Serve as a preceptor for dietetic interns, overseeing project-based learning and supporting a culture of mentorship.
Additional (unranked in percentage, but integrated throughout workday):
Program Data, Grant Tracking & Evaluation
Maintain accurate tracking of program activities tied to grant funding and reporting requirements.
Collaborate with internal and external teams to ensure high-quality data collection and impact evaluation.
Use data and feedback to continuously improve program delivery and effectiveness.
Public Health Best Practices & Equity Focus
Stay current on emerging practices in Food is Medicine, nutrition security, and public health.
Apply a health equity lens to all programming, ensuring services are inclusive, culturally relevant, and community-informed.
Other duties as assigned
When & Where:
- Schedule: Monday – Friday, 8:00 AM – 4:30 PM.
Occasional evenings and/or weekends may be required for meetings or special events as well as responding to issues, outside of normal working hours (e.g., disaster response, community events, etc.).
- Travel: Occasional daytime travel required, up to 10 -15% of work week within the local service area, and to and from the Main Distribution Center in Nashville, Tennessee as needed.

WorkFlex arrangements are complex and specific to individuals and roles.
- Essential business/ first responder: In instances of a federal, state or locally declared emergency, Second Harvest is typically considered an essential business service and emergency responder. All employees may be required to perform regular or emergent duties on-site. Employee safety is balanced with business and community necessity. Some positions are more likely to be required on-site than others.
Is this you?
Experience: 3+ years of related experience and/or training.
Skills & Capabilities:
Master’s degree in Public Health, Nutrition, Health Education, or related field preferred.
3–5 years of experience in community health, nutrition programming, or nonprofit program implementation.
Strong organizational and time management skills; ability to manage multiple priorities.
Demonstrated experience with program data tracking, outcome reporting, and grant deliverables.
Familiarity with program evaluation methods, research execution, and ethical data practices, including working with external evaluators or academic partners.
Experience in maintaining professional relationships with healthcare or clinical partners.
Knowledge of healthcare partnerships, health equity, and social determinants of health.
Excellent communication skills and ability to build relationships with diverse stakeholders.
Commitment to advancing equity, inclusion, and dignity in food access.
Competencies: Coaching and developing others, Result orientation and execution, Quality improvement, Project management.
Other requirements: Our pre-hire process requires acceptable results from the following: professional reference feedback, criminal background, and Motor Vehicle Record. Current Driver’s License and automobile insurance are also required. Criminal backgrounds are evaluated on a case-by-case basis considering several factors (timing, frequency, severity and job relation).
